One face. One needle depth. One setting. ❌
If you’re using the same microneedling depth across the entire face, you’re not really individualizing the treatment.

Why?

Because facial anatomy isn’t uniform.

Periorbital area ≠ forehead ≠ nose ≠ cheeks.

And depth shouldn’t be decided by a chart alone.

It should be based on:

🔬 Skin/tissue characteristics
🎯 Treatment indication
👤 Individual patient factors
📍 Treatment zone
⚙️ Device characteristics
✅ Desired clinical endpoint

The depth you consider for skin rejuvenation cannot simply be copied for acne-scar treatment.

And this is where many practitioners make the mistake:

They learn a number instead of learning how to make the decision.

There is no magic “one depth for the whole face” rule.

“It’s just a little serum. What’s the harm?”
Microneedling deliberately disrupts the skin barrier and creates microscopic channels. So the composition, sterility and intended use of anything applied around the procedure matters.
A serum formulated for the skin’s surface is not automatically safe to introduce into deeper tissue.
A systematic review of the published literature identified 15 patients across 13 studies with non-necrotizing granulomatous inflammation following microneedling. Motorized pen use and topical vitamin C application were implicated in a majority of reported cases. Delayed-type hypersensitivity was commonly proposed as the mechanism. Response to treatment was inconsistent, and some cases proved treatment-resistant.
Fifteen patients in the entire literature. This is rare, and I want to be accurate about that.
But rare and serious is still worth knowing — because these reactions don’t always appear immediately, and they don’t always resolve easily.
My rule is simple: during the procedure, use only what is appropriate in composition, sterility and intended use.
“Thoda sa serum lagane se kya hoga” is exactly the thinking clinical practice should challenge.

Before I blame the machine, I ask one question: what was the scar, actually?

Rolling, boxcar and ice pick have different architecture — and different responses. Ice pick scars in particular tend to show limited response to microneedling alone.

Most “it didn’t work” cases are really “wrong scar type” cases.

Assessment first. Machine second.
