Operated upon a 48 year old person , with a sialolith in the left submandibular duct.
The patient has history of stroke with paralysis and was on Antiplatlets for the past 5 years.
Patient had severe TRISMUS with only 2 finger mouth opening ( which increased to 2.5 fingers after relaxant). In every other institution , gland excision was the only option given to the patient. Patient came to me only because of sialendoscopy as he heard that gland could be saved in this procedure and he didnt want an external incision.
As soon as the GA was given, the stone slided back, making it difficult to remove it.
Sialendoscopic assisted stone removal with internal combined approach was done.
Patient was happily discharged next day.
All the antiplatlets were started next day post surgery.

A 5 mm stone was removed by basketing the stone through sialendoscopy.
A beautiful surgery, painless and sutureless.
Pt was discharges happy and healthy with being able to resume work from 2 nd day of surgery.

Patient came with left sided submandibular swelling. A 4.7 mm stone was visualised in the duct on a CT scan. Combined approach with basket removal was done. ANOTHER GLAND WAS SAVED. 😇
