On this day devotees keep fast, decorate temples with flowers and lights, and sing bhajans. At 12 o'clock at night, the idol of baby Krishna is placed in a beautifully decorated cradle (jhula) and worshipped. It is celebrated with great joy in Mathura, Vrindavan and Dwarka.

Lord Krishna gave the message of Karma through the Bhagavad Gita - "Do your duty without worrying about the result". Janmashtami inspires us to follow the path of truth, love and dharma.

* Treatment & Medication - Key Points*

COPD = Chronic Obstructive Pulmonary Disease. फेफड़े की नली सिकुड़ जाती है, हवा फंस जाती है। ठीक नहीं होता, पर दवा से control होता है।

*1. इलाज का Goal*
1. सांस फूलना कम करना
2. Exacerbation/दौरे रोकना
3. Life quality सुधारना
4. बीमारी को धीरे बढ़ाना

*2. सबसे जरूरी: Smoking बिल्कुल बंद*
बीड़ी, सिगरेट, हुक्का, चूल्हा धुआं - सब बंद। दवा तब काम करेगी। ये 50% इलाज है।

*3. Main Medicines - Inhalers First Line हैं*

*A. Rescue/Reliever - जरूरत पड़ने पर*
- *Salbutamol Inhaler* - Asthalin, Levolin
2 puff लो जब सांस फूले। 5 min में आराम। दिन में max 8 puff।

*B. Maintenance - रोज लेना है, भले ठीक लगो*
1. *LAMA - सबसे जरूरी*
*Tiotropium* - Tiova, Spiriva 18mcg Rotacap रोज 1 बार सूंघो
24 घंटे नली खुली रखता है।

2. *LABA*
*Formoterol, Salmeterol* - Foracort, Seroflo
12 घंटे आराम देता है।

3. *ICS - Steroid*
*Budesonide, Fluticasone* - Foracort, Seroflo में मिला रहता है
सिर्फ तब जब साल में 2+ बार भर्ती होना पड़े। कुल्ला करना जरूरी।

*C. Combo Inhaler - ज्यादातर को यही देते हैं*
1. *LAMA + LABA*: Ultibro, Anoro, Brimica - रोज 1 puff। Moderate-Severe में best।
2. *LABA + ICS*: Seroflo 250, Foracort 200 - दिन में 2 बार।
3. *Triple Therapy*: Trelegy, Triohale - LAMA+LABA+ICS तीनों। जब बार-बार दौरा पड़े।

*4. Tablets जो साथ में चलती हैं*
1. *Doxophylline 400mg* - Doxovent रात में 1। नली खोलता है।
2. *NAC 600mg* - Mucinac दिन में 2 बार। बलगम पतला करे।
3. *Azithromycin 250mg* - हफ्ते में 3 दिन। बार-बार infection रोकने को।
4. *Roflumilast 500mcg* - Severe bronchitis type में।

*5. दौरा पड़ने पर - Acute Exacerbation*
1. *Nebulization*: Duolin + Budecort respule हर 6 घंटे
2. *Tab Prednisolone 40mg* सुबह 5 दिन के लिए
3. *Antibiotic*: Augmentin 625 या Azithro 500, 5 दिन
4. *Oxygen*: SpO2
