ALT AST Enzymes, How To Interpret Liver Injury Patterns Correctly!
ALT and AST are not liver function tests. They reflect hepatocyte injury. Understanding the pattern helps prevent wrong lab interpretation.
🔬 What ALT and AST Show
Know the source before reading the report
1️⃣ ALT is more liver specific, rises in viral hepatitis and fatty liver
2️⃣ AST is present in liver, muscle, heart, so context matters
3️⃣ Enzyme elevation means cell damage, not function loss
📊 AST ALT Ratio Guide
Simple clinical clues from enzyme patterns
1️⃣ ALT higher than AST, think viral or metabolic injury
2️⃣ AST ALT ratio above 2, consider alcohol related damage
3️⃣ Mild equal rise may appear in chronic liver disease
⚠️ Clinical Tip
Always correlate with bilirubin, ALP, GGT, history, and symptoms.

🟡 1. Fatty Liver (Hepatic Steatosis)
🔹 What it is:
A condition where extra fat builds up inside liver cells.
🔹 Common Causes:
Obesity
Diabetes
High cholesterol
Alcohol use
Poor diet (junk food, sugar, fried foods)
Lack of exercise
🔹 Stages:
Simple Fatty Liver (NAFLD) – fat present but no damage
Fatty Liver with Inflammation (NASH) – fat + inflammation starts damaging liver
🔹 Symptoms:
Usually no symptoms at first
Sometimes:
Tiredness
Mild pain in upper right abdomen
Weakness
🔹 Is it reversible?
✅ YES — completely reversible with:
Weight loss
Exercise
Healthy diet
Controlling sugar & cholesterol
Avoiding alcohol
If treated early, liver can become normal again.
🔴 2. Liver Cirrhosis
🔹 What it is:
A late-stage liver disease where:
➡️ Normal liver tissue is replaced with permanent scar tissue (fibrosis)
The liver becomes:
Hard
Shrunken
Unable to work properly
🔹 Causes:
Long-term fatty liver disease
Chronic alcohol use
Hepatitis B or C
Untreated NASH
Long-term liver inflammation
🔹 Symptoms:
Swollen abdomen (ascites)
Yellow skin/eyes (jaundice)
Weight loss
Vomiting blood
Swollen legs
Confusion (hepatic encephalopathy)
Easy bruising
Severe fatigue
🔹 Is it reversible?
❌ NO — cirrhosis is permanent
Damage cannot be undone. Treatment only:
Slows progression
Prevents complications
In advanced cases → Liver transplant may be needed

Parathyroid Disorders made SIMPLE 🩺
Too much PTH?
➡️ Calcium leaves the bones and enters the blood
Too little PTH?
➡️ Calcium drops and muscles become EXCITABLE
Remember:
📈 Hyperparathyroidism = “Bones, Stones, Groans, Thrones, Psychiatric Overtones”
📉 Hypoparathyroidism = Tetany + Tingling + Seizure risk
NCLEX TIP 🚨
If your post-thyroidectomy patient has:
⚡ Tingling lips
⚡ Muscle spasms
⚡ Positive Chvostek’s or Trousseau’s sign
Think 👉 LOW CALCIUM = HYPOparathyroidism
