"Sguardo stanco, appesantito, le borse, la palpebra che cade sulla rima ciliare, la difficoltà a truccarsi. Quante volte le mie pazienti arrivano in studio con uno di questi problemi. Questa signora li aveva tutti e tre."

In un caso così l'indicazione è chirurgica: blefaroplastica superiore e inferiore. Ma fatta in modo conservativo. L'obiettivo non è togliere: è ricreare la situazione che c'era prima, senza scavare, senza infossare l'occhio, senza cambiarne la forma.

Sopra ho fatto una trasposizione del grasso: non l'ho rimosso, l'ho spostato dove mancava. Per questo la palpebra superiore resta piena, che è la caratteristica di un occhio giovane.

Sotto ho tolto le borse per via transcongiuntivale, cioè dall'interno della palpebra, senza incisioni sulla pelle, e ho fatto un lipofilling del solco. La borsa non c'è più, ma non c'è nemmeno l'occhio scavato che a volte la sostituisce.

Il risultato è armonioso, e la paziente lo riconosce come suo. Le foto che vedete sono di questo singolo caso: ogni sguardo ha la sua diagnosi.

"A tired, heavy look, under-eye bags, an upper lid resting on the lash line, make-up that will not stay on. My patients come to me with one of these problems all the time. This lady had all three."

In a case like this the answer is surgical: upper and lower blepharoplasty. But a conservative one. The goal is not to remove tissue, it is to restore what was there before, without hollowing the eye, without a sunken look, without changing its shape.

On the upper lid I performed a fat transposition: the fat was not removed, it was moved to where it was missing. That is why the upper lid stays full, which is what a young eye looks like.

On the lower lid I removed the bags through a transconjunctival approach, from inside the eyelid, with no skin incision, and added fat grafting to the tear trough. The bag is gone, and so is the hollow that sometimes replaces it.

The result is harmonious, and the patient recognises it as her own. The photos show this single case: every pair of eyes needs its own diagnosis.

"Pensate già adesso, che è estate, alla cura della vostra pelle per il prossimo autunno e il prossimo inverno."

Lo dico a settembre perché è a settembre che si decide. I trattamenti sulla superficie della pelle, il laser prima di tutti, si fanno in autunno e in inverno: dopo la seduta il sole può fare più danni di quanti benefici abbia dato il trattamento.

C'è poi un dato pratico del mio studio: la lista d'attesa è di qualche mese. Chi pensa di fare il laser a ottobre e chiama a ottobre, la prima seduta la fa a dicembre. Chi prenota adesso, la fa nella finestra giusta.

Vale per il viso e per il corpo: macchie su mani e décolleté, smagliature, cicatrici, lassità di braccia e addome si trattano nei mesi in cui la pelle guarisce senza sole.

In primavera, forse, solo con una schermatura totale, e con qualche preoccupazione da parte mia. Quando si parla di pelle, la parola è pianificazione.

"Think now, while it is still summer, about how you will care for your skin this autumn and winter."

I say it in September because that is when the decision is made. Treatments on the surface of the skin, laser first of all, belong to autumn and winter: after a session the sun can do more harm than the treatment did good.

There is also a practical fact about my practice: the waiting list runs a few months. If you plan a laser treatment for October and call in October, your first session will be in December. If you book now, it falls inside the right window.

This applies to the face and to the body: sun spots on hands and décolleté, stretch marks, scars, laxity on arms and abdomen are treated in the months when the skin heals without sun.

In spring, perhaps, only with total sun protection, and with some concern on my part. When it comes to skin, the word is planning.

"Se i tessuti in eccesso ci sono, per me il lifting deep plane resta la soluzione più idonea. Se non ci sono, oggi posso fare molto meno danno."

Dopo la diagnosi una sola domanda decide la strada: quanta cute in eccesso c'è.

Quando alla discesa dei tessuti si somma un eccesso vero, il lifting è l'intervento di riferimento, per un motivo geometrico. Il vettore del lifting è prevalentemente obliquo, porta i tessuti verso l'orecchio, e dopo sette o otto centimetri quella cute in più va rimossa.

Se l'eccesso non c'è, o è minimo, lo stesso riposizionamento lo ottengo con un insulto chirurgico molto più piccolo. I fili permanenti lavorano dove sta il problema: solchi nasogenieni, marionetta, linea mandibolare. Nel piano profondo, sotto lo SMAS, riagganciano la zona centro-facciale e riportano su i tessuti, bande platismatiche comprese.

Con un eccesso moderato imposto un vettore un po' più obliquo e rimuovo la cute eccedente, senza alcuno scollamento.

Non è una gara fra tecniche: sceglie la quantità di cute in eccesso.

"When the tissues really are in excess, a deep plane facelift remains, in my view, the most appropriate option. When they are not, today I can do far less damage."

After the diagnosis, one question decides the route: how much excess skin is there.

When tissue descent comes with genuine excess skin, the facelift is the reference procedure, for a geometric reason. The lifting vector is mainly oblique, it carries the tissues towards the ear, and after seven or eight centimetres that extra skin has to be removed.

When there is no excess, or very little, I obtain the same repositioning with a far smaller surgical insult. Permanent threads work where the problem is: nasolabial folds, marionette lines, jawline. In the deep plane, below the SMAS, they re-anchor the mid-face and lift the tissues back up, platysmal bands included.

With a moderate excess I set a slightly more oblique vector and remove the excess skin, with no undermining at all.

This is not a contest between techniques: the amount of excess skin chooses.

Sui fili circolano ancora molte informazioni imprecise. Rispondo alle tre domande che ricevo più spesso.

I fili danno un risultato? Sì, se sono posizionati nel piano corretto. Dipende dall'esecuzione, non dallo strumento.

Come faccio a sapere di essere sotto lo SMAS, che è sottilissimo? Me lo dicono i legamenti di ritenzione, che stanno nello SMAS e al di sotto: quando li attraverso avverto un click netto. È la conferma che sono nel piano giusto, ed è lì che posiziono il filo.

E la pelle in eccesso? Il cedimento non è un allungamento: i tessuti non si sono allungati, sono scesi di posizione. Quando un eccesso cutaneo c'è davvero, minimo o moderato, lo tratto con un micro-lifting associato ai fili: è l'HDS Thread Lifting Plus.

Serviranno ancora anni perché più colleghi la conoscano. Ma ha già la sua indicazione, e non è un ripiego.

There is still a lot of imprecise information about suspension threads. Here are the three questions I am asked most often.

Do threads deliver a result? They do, when they are placed in the correct plane. It depends on the ex*****on, not on the device.

How can I be sure I am below the SMAS, given how thin it is? The retaining ligaments tell me. They sit within the SMAS and beneath it, and as I pass through them I feel a distinct click. That is the confirmation I am in the right plane, and that is where the thread goes.

And excess skin? Tissue descent is not stretching: the tissues have not lengthened, they have moved down. When there genuinely is excess skin, mild or moderate, I address it with a micro-lift combined with the threads: HDS Thread Lifting Plus.

It will take a few more years for more colleagues to know it. But it already has its own indication, and it is not a fallback.

"Il trattamento più adatto alla persona che ho di fronte non
è necessariamente il trattamento migliore in assoluto."

E non è nemmeno, quasi mai, il trattamento che preferirei io.
Dopo la diagnosi c'è un secondo passaggio che conta quanto il
primo: capire chi ho davanti e che percorso può affrontare.

C'è chi accetta volentieri un miglioramento parziale, purché
la ripresa sia breve e nessuno se ne accorga. C'è chi ha un
budget preciso, o chi non vuole settimane di recupero. Non
sono capricci: sono vincoli reali, e li considero parametri
della scelta clinica.

Un esempio. Un cedimento iniziale della linea mandibolare si
può affrontare con un lifting, anche deep plane. Ma a volte i
tessuti non si sono allungati: sono scesi perché nelle logge
di grasso della guancia c'è meno volume di prima. Lì la strada
può essere un filler o un lipofilling.

Prima la diagnosi, poi la scelta del percorso insieme alla
persona. Non necessariamente partire a bomba.

"The most appropriate treatment for the person in front of me
is not necessarily the best treatment in absolute terms."

And it is almost never the one I would personally prefer.
After the diagnosis comes a second step that matters just as
much: understanding who the patient is and what kind of path
they can realistically take.

Some people are happy with a partial improvement, as long as
recovery is short and nobody notices. Others have a set
budget, or cannot afford weeks of downtime. These are not
whims: they are real constraints, and I treat them as part of
the clinical decision.

An example. Early jowling can be addressed with a facelift,
deep plane included. But sometimes the tissues have not
stretched: they have descended because the cheek fat
compartments have lost volume. In that case filler or fat
grafting may be the answer.

Diagnosis first, then the choice of the path together with
the patient. There is no need to go in all guns blazing.

«Se mi chiedi di essere tirata come la pelle di un tamburo,

Lo dico subito in visita, perché è una questione di
indicazione. Una trazione molto marcata si ottiene con un
lifting chirurgico deep plane: è un altro intervento, con un
altro obiettivo.

L'HDS Thread Lifting nasce per il risultato opposto: la
naturalezza. I fili permanenti lavorano in profondità, sotto lo
SMAS, e da lì riposizionano i tessuti di superficie in modo
morbido. Il viso sale, ma la sua mobilità resta la stessa: il

«Funziona perché mette insieme due cose che ho imparato
separatamente: il lifting deep plane e i fili.»

Per vent'anni ho operato lifting chirurgici lavorando sul piano
profondo, quello dello SMAS. In parallelo inserivo fili di trazione,
prima riassorbibili e poi permanenti. Per anni sono rimaste due
strade separate: la logica della chirurgia profonda da un lato, i
fili dall'altro.

Quello che mancava per farle incontrare era il filo. L'ho trovato in
un filo con un'anima in poliestere, che non si allunga, rivestita di
silicone solido e liscio, e con una struttura tridimensionale a
conetti che ancorano i tessuti senza pungerli.

Con quel filo ho potuto portare la logica del deep plane dove prima
i fili non arrivavano: in profondità, senza aprire una via
chirurgica per arrivarci.

Non è lo strumento a fare la tecnica. È il piano in cui lo si mette.

"It works because it brings together two things I learned
separately: deep plane surgery and threads."

For twenty years I have performed surgical facelifts working on the
deep plane, the SMAS layer. Alongside that, I placed traction
threads — absorbable first, then permanent ones. For a long time
these stayed two separate paths.

What was missing was the thread itself. I found it in a thread built
around a polyester core that does not stretch, coated in solid,
smooth silicone, with a three-dimensional cone structure that
anchors the tissues without piercing them.

That thread let me take the deep plane logic where threads could not
reach before: deep, without opening a surgical access to get there.

The instrument does not make the technique. The plane you place it
in does.

«Il silicone non è un materiale solo: dire "silicone" senza dire quale non significa niente.»

Mi chiedono spesso di cosa sono fatti i fili. Dietro la domanda c'è quasi sempre una diffidenza verso il silicone — e la capisco: da
paziente l'avrei anch'io, se non sapessi come sono fatti.

Il silicone che ha creato problemi è quello liquido, iniettabile:
causava la formazione di granulomi, come tutti gli iniettabili
permanenti, ed è vietato in Italia dagli anni Novanta. L'altro caso
critico è il silicone ruvido, per esempio quello di certe superfici
protesiche: proprio perché ruvido può indurre una reazione
infiammatoria.

Il rivestimento dei fili che utilizzo è invece silicone solido e
liscio: né liquido né ruvido. È un materiale biologicamente inerte,
lo stesso impiegato da oltre sessant'anni nelle protesi d'anca e di
ginocchio.

Quello che conta non è il nome del materiale. È il suo stato fisico
e la sua superficie.

"Silicone is not one single material: saying 'silicone' without
saying which one means nothing."

Patients often ask me what the threads are made of. Behind that
question there is almost always a distrust of silicone — and I
understand it: as a patient I would feel the same, if I didn't know
how the threads are built.

The silicone that caused harm is liquid, injectable silicone: it led
to granulomas, as every permanent injectable did, and it has been
banned in Italy since the 1990s. The other critical case is textured
silicone, such as some implant surfaces: precisely because it is
rough, it can trigger an inflammatory response.

The coating of the threads I use is solid and smooth silicone:
neither liquid nor rough. It is a biologically inert material, the
same one used for over sixty years in hip and knee prostheses.

What matters is not the name of the material. It is its physical
state and its surface.

«Dovrei fare un lifting cervico-facciale a mia figlia di 19 anni?»

È la domanda che mi sono posto mettendo alla prova un gesto che tantissimi pazienti fanno in studio: tirarsi la pelle del viso per mostrarmi "quanto cede". Per anni quel gesto ci ha portato dritti verso la stessa risposta: il lifting cervico-facciale.

L'ho confrontato su tre generazioni della mia famiglia — io (52 anni), mia figlia (19), mia madre (78) — e lo spostamento della pelle è risultato quasi identico nei tre casi. Se fosse davvero eccesso cutaneo, su mia figlia non dovrebbe spostarsi quasi nulla.

La verità è che l'invecchiamento del viso spesso non è (solo) una questione di pelle in eccesso: è una ptosi dei tessuti, cioè un abbassamento della struttura di sostegno, a volte con eccesso cutaneo, a volte senza. Capire quale dei due meccanismi prevale è il primo passo di ogni diagnosi corretta.

"Should I perform a facelift on my 19-year-old daughter?"

That's the question I asked myself after testing a gesture I see all the time in my patients: pinching the skin of the face to show me "how much it's sagging." For years, that gesture has led all of us straight to the same answer — a cervicofacial facelift.

I compared it across three generations of my own family — myself (52), my daughter (19), my mother (78) — and the skin moved by almost the same amount in all three. If it were really excess skin, it shouldn't move much at all on my daughter.

The truth is that facial aging is often not (only) about excess skin: it's tissue ptosis — the supporting structures sliding downward — sometimes with excess skin, sometimes without. Understanding which mechanism is at play is the first step of any correct diagnosis.

«Il denominatore comune resta sempre lo stesso: causare il minor trauma
chirurgico possibile.»

Uso l'HDS Thread Lifting — High Definition SMAS Thread Lifting — da quasi
quattro anni: fili permanenti posizionati sotto lo SMAS, cioè lo strato
profondo che sostiene i tessuti del viso.

È un concetto che nel tempo si è evoluto, ed è diventato HDS Thread Lifting
Plus: ai fili affianco il fat grafting, i dispositivi per il sottocute come
Ignite, oppure un microlifting — un'incisione molto limitata con cui
riposiziono i tessuti verso l'orecchio e rimuovo la cute in eccesso.
