QUESTION :7 ALL ABOUT PAEDIATRIC HIV AND MOLLUSCUM

Hello please help me my niece who is HIV positive has developed a bad rash on the face and the growth, also called โ€œEnsuundoโ€ in my local language,are almost obliterating one of
her eyes.what can we do now because the child is refusing to go to school as others tease her.?

RESPOND (ANSWER)

HELLO my friend. First sorry for taking long-time without response, In your Question there is some word in your language that is supposed to known and now I get it.....What's you describe here is what is called โ€œmolluscum contagiosumโ€ in medical language. It is caused by a virus and appears in clusters, which are smooth and waxy, commonly at the face but they can occur anywhere on the body except on the palms and soles of the feet. Most of this swellings normally disappear in six to night month(but if you are very careful I hope or 3-4 months)and treatment is usually for consmetic reasons because ad you rightly mention they disfigure the face . And I hopefully some Doctors can remove them physically, a process known as curettage,or by applying chemicals, which can make them disappear (feew inu ndikumati kamofoleji muli phee amalawi)

However since the child has HIV and immune suppressed the rash may not respond well to the above-mentioned measures. She should be assessed and put on ARVs. As the child's immunity improves, the rash and any other opportunistic infections will respond to treatment and eventually disappear.(please take not of it and understand me very well)

Lastly,if you are HIV positive and you have children, or anyone else who is looking after children whose parents are known or suspected to die due to HIV and AIDS, make sure that these children are tested for HIV. Then if they are positive they can be helped before they develop symptoms, by which time it may be too late to help them. HIV infection in children can now be diagnosed as early as during the first month after delivery, so there is no need to wait until children are one and a half years old as was the practice in the past (please apa ponkha mukanandivako abalee...!!

Hello bigy**** I have started taking ARVs on the advice of my doctor and after being on them for two months, I feel okay. However I have a problem because I enjoy drinking beer and cannot sleep without it. Is drinking beer dangerous when I am on ARVs..? Am Martin from Uganda

Okay fyn broo.. It is good that you are feeling fine after two months on ARVs. This is important because ARVs are to make the quality of your life better. Like all drugs however, ARVs have their restrictions. This means you have to consider changing some of your habits and practices in order to continue enjoying a better quality of life on ARVs.

An important lifestyle choice for any person living with HIV is dealing with any form of substance abuse of include alcohol consumption. Addiction to any substance in itself can be life threatening and the fact that you can not sleep without taking alcohol suggests that you are suffering from a certain degree of addiction.if you have to kick the habit you may require professional help.(kaya ukundimva mu Uganda iwe)

It is important to understand why wakuchipatala the Heath insist that people living with HIV (PLHIV) should abstain from taking alcohol. Abuse of alcohol can lead to damage to organs like the brain and liver, which are usually attacked and damaged by HIV. ARVs cause liver toxicity (but I don't mean stop taking it aa iwe mbuu) and so alcohol, which is associated with damage of the liver ,is Best avoided when one is on ART.

Alcohol consumption also leads to riskier s*xual behavior that could expose you to getting infected with new and maybe more virulent strain of HIV (aka kokha ndabera ndipitabe mu dictionary ndikaziwe mmmh) which could make your condition deteriorate faster. You could on other hand infect other people with HIV if you indulge in unprotected s*x, which often happens when people are intoxicate with alcohol. Remember, you shall never overcome this disease if you who are already infected are not vigilant in preventing new infections.

Lastly, taking alcohol may make adherence to ARVs difficult because it's easy to forget to take the drugs at the right time and in the right amount when you are drunk. This will mean the virus is not being maximally suppressed and may even become resistant to the ARVs you are on . Therefore you are better off if you do not take alcohol while you are on ART.

HELLO BOSS**** I don't know how..! But I have just found out that I am HIV positive and I am scared because I do not wait to die. I have heard that ARVs help but then the many restrictions they come with,eg. You must eat before taking to them, plus the side effects, make me feel life on ARVs is hell.!! I am also told that the drugs have to be stored in fridge and at the moment I don't have one. Please help!!! Am Mohammed from Neno district.

Wow thank you for this question and for expressing your fear about ARVs. Remember the Cardinal goal of ART is to make quality of life for people living with HIV (PLHIV) better. All drugs, ARVs inclusive, have side effects but we use them just because the benefits out weigh the unwanted side effects. The other thing you must know is that not all HIV Positive people have to be on ARVs. Some people whose immune system is still strong and whose CD4 coun remain high do not need to start on ART yet. Therefore you need to go to center or hospital where your CD4 count can be done so that you know how strong your immune system is before you start on ART.

Some of fear you express about ARVs are actually myths. Should your doctor advise you to start on ART, you will undergo comprehensive pre-ART and adherence counseling which will dispel most of your fears(let's add my number for adherence mwina 0882070474/0980174150). In fact, most of first line drugs in use have no serious food restrictions and and do not need to be stored in a fridge..
