They usually occur during the first few days of menstruation when bleeding is heaviest, causing blood to accumulate in the uterus and clot.

Key Causes and Factors:
Heavy Menstrual Flow (Menorrhagia): When bleeding is rapid or excessive, the body's natural anticoagulants cannot keep up, resulting in clots.
