It's driven by hormones, which is why it flares with periods, pregnancy, and perimenopause, and why stress can trigger it too.

Because melasma is hormonally driven, it can come back even after successful treatment, if those hormonal triggers are still present. So if a therapist tells you it "won't come back," that's simply not accurate — melasma is something you manage long-term, not something you cure once and forget.
