Frequently asked questions.
1. Pwede po ba akong mag-enroll kahit hindi pa ako nakapag-Pre-Enrollment Medical and Dental Examination (PEMDE) at wala pang Medical Certificate mula sa Infirmary?

- Oo, pwede ka pa ring mag-enroll. Pumunta lamang sa iyong assigned enrollment schedule na ibinigay ng Admission Office. (Except sa mga freshmen ng BS Criminology, na kinakailangang makumpleto muna ang Pre-Enrollment Medical and Dental Examination (PEMDE) bago mag-enroll, as required by their college.)

2. Hindi po ako nakapunta sa assigned schedule ng aming college para sa Pre-enrollment Medical and Dental Examination (PEMDE). Hindi na po ba pwedeng mag pa-PEMDE sa ibang araw?

- Pwede ka pa rin magpa-PEMDE. Hintayin lamang ang bagong schedule na ia-announce namin para sa mga freshmen na hindi nakapagpa-medical at dental examination sa kanilang assigned schedule.

Reminders
• Bring all complete requirements and one (1) valid ID.
• Observe proper personal hygiene (clean attire, oral hygiene, etc.).
• Wear comfortable clothing and bring your own pen.
• Eat a light meal before the medical examination.
• Bring a photocopy of your health records, if available.
General Requirements
• Accomplished Student Health Record
• One (1) long ordinary folder with plastic cover/jacket
• One (1) 2x2 ID picture
• One (1) fastener
• Official Receipt for the Php 50.00 Medical Certification Fee
• Medical Certificate/Clearance from the attending physician for enrollees with pre-existing medical conditions

You may also reproduce the form as needed.
