Severe Septal Deviation | Septal Repositioning

This case presented with a severe septal deviation, requiring careful correction to restore a more functional and balanced nasal framework.

One of the techniques utilized was caudal septal repositioning using the Swinging Door technique a maneuver where the deviated caudal septum is mobilized, repositioned toward the midline, and secured in a more stable and functional position.

Osteotomy can also play an important role when the bony nasal framework contributes to the deviation. By carefully mobilizing and repositioning the nasal bones, it helps improve alignment and allows the entire nasal framework not just the septum to be addressed.

However, achieving a perfectly straight septum and ideal external result is not always possible. The underlying bony septum, cartilage memory, tissue thickness, and severity of the original deformity can all influence the final outcome.

Good results require more than technique alone. They require proper anatomical knowledge, appropriate surgical armamentarium, meticulous ex*****on, and realistic communication with the patient regarding what can and cannot be achieved.

The goal is not simply to make the nose straighter it is to create the best possible functional and aesthetic result within the limitations of the patient’s anatomy.

At this stage, much of the swelling has subsided, allowing the nose bridge and tip to become more refined while maintaining a natural-looking result. Although healing is well underway, subtle changes will continue over the coming months as the tissues settle further.

Every nose heals at its own pace, and patience is an important part of achieving the final outcome.
