✔️ Accomplished Application Form (Registrar’s Office)
✔️ Photocopy of Grade 6 Report Card (Gen. Avg. 85, no final grade below 83 in all subjects)
✔️ PSA Birth Certificate

📝 Assessment Schedule:
📚 Aptitude Test (English, Math, Science): April 30, 2026 (8:00–11:00 AM)

🎨 Talent Screening: April 30, 2026 (1:00–4:00 PM)
