Bacteria are becoming resistant to antibiotics faster than we can develop new ones. ๐Ÿฆ  The more we use antibiotics, the less effective they become, which is a huge threat to public health.

Research shows that at least HALF of the antibiotics prescribed are NOT necessary โ€“ that is, the illness did not require a prescription antibiotic! ๐Ÿ˜ณ Why is this happening? The literature suggests several reasons:

First, when a patient comes in sick, many doctors feel as though families expect tangible actions to address their concerns. In other words, parents donโ€™t want to leave the office โ€œempty handed.โ€ ๐Ÿ‘ I've seen families become upset after hearing that their child's illness is due to a virus and therefore they do not need antibiotics. A familyโ€™s angst is often expressed in the form of low patient satisfaction scores used to measure physician "performance." ๐Ÿ“ This is just an unfortunate set-up all around. It does NOT inspire medical professionals to stand their ground.

Secondly, bacterial and viral infections often have overlapping symptoms. Physicians admit that uncertainty surrounding the diagnosis can lead to antibiotic prescribing as the perceived โ€œsafe choice.โ€ While it is often harmless to โ€œwait and seeโ€ how an illness will pan out (with good return precautions), parents may perceive delayed antibiotic treatment as an inconvenience or even a โ€œmissed diagnosis."

Lastly, some would argue that doctors are pressed to see too many patients in wayyy too short of time. It is exhausting, if not impossible due to time constraints, to convince 30 families per day that their cough/fever/sore throat is viral and therefore does not require antibiotics. Even when the appropriate reassurance IS provided, some parents (who are rightfully desperate to make their child feel better) will seek out additional opinions until they get the prescription that they are yearning for. ๐Ÿ˜ž The child will then receive an unnecessary antibiotic and their symptoms will begin to subside โ€“ not because the antibiotic is โ€œworkingโ€ - but because thatโ€™s the natural course of a viral illness. I call this the "confirmation bias virus." Misperceptions are reinforced, and the cycle of unnecessary antibiotic treatment ensues.

So what can you do to help fight antibiotic resistance? Educate and empower yourself just like you are doing right now. ๐Ÿ™Œ Try to resist the urge to rush to the doctor every time that your child has symptoms of cough, cold, congestion, and fever. It may bring revenue to the doctorโ€™s office, but it wastes YOUR time and resources. If you feel as though your child's illness warrants a trip to the emergency room, try to select a Pediatric ER, as adult ER physicians may not feel as comfortable providing you with the reassurance that you need.

Remember, the common cold (cough, congestion, runny nose, fever, sore throat, and/or poor appetite) is caused by a viral infection. Antibiotics will not help. ๐Ÿ’Š Antibiotics only treat bacterial infections.

Fever is NOT the enemy โ€“ dehydration and breathing difficulties are the enemies. As long as your child doesnโ€™t appear ill and is otherwise healthy above the age of 3 months, you can try at-home techniques such as nasal saline, bulb suction, humidifiers, salt water gargles, Tylenol, Vicks v***r rub, etc.

Nursing lines can sometimes be helpful. Some lines are fantastic whereas some seem to tell nearly everyone to come into the office. This is just one example of how liability issues ruin everything. ๐Ÿ™„ If you express concern over the phone, you will likely be told to โ€œcome on in.โ€ Instead, try to specifically ask for AT-HOME tips and suggestions for the symptoms that are bothering your child the most. This way, the person on the other line may feel comfortable giving you advice from afar.

Yes, this is me telling you how it is. Iโ€™m a medical minimalist at heart โ€“ a lot of pediatricians are. โš•๏ธ Find a doctor that saves you from unnecessary testing and treatment as often as possible.

PLEASE donโ€™t get me wrong: It is ultimately 100% your doctor's job to limit the number of antibiotic scripts that they write. โ€ผ๏ธ I'm just encouraging you to be open-minded when your doc takes the time to explain the difference between a viral infection (that will get better on its own) versus a bacterial infection that requires the use of antibiotics. Hopefully what you've read here will make the process go a little bit smoother.

While doing my OPD consult, a lola came in for a check-up โ€”not for any specific complaint, but to ask for a prescription for an herbal capsule she had been taking for MALASAKIT claims. She had been using it for two months, convinced by a radio advertisement that it could cure almost everythingโ€” hypertension, diabetes, ulcers, hepatitis, pneumonia, kidney problems, and even HIV.

She confidently told me:
"๐พ๐‘Ž๐‘›๐‘– ๐‘Ÿ๐‘Ž ๐‘Ž๐‘˜๐‘œ, ๐‘‘๐‘œ๐‘, ๐‘˜๐‘Ž๐‘ฆ ๐‘œ๐‘Ÿ๐‘”๐‘Ž๐‘›๐‘–๐‘ ๐‘ก๐‘Ž๐‘๐‘œ๐‘  ๐‘‘๐‘–๐‘™๐‘– ๐‘š๐‘Ž๐‘˜๐‘Ž-๐‘‘๐‘Ž๐‘œ๐‘ก ๐‘ ๐‘Ž ๐‘˜๐‘–๐‘‘๐‘›๐‘’๐‘ฆ."

"๐‘€๐‘Ž๐‘  ๐‘š๐‘ข-๐‘Ž๐‘›๐‘”๐‘Ž๐‘ฆ ๐‘š๐‘Ž๐‘› ๐‘˜๐‘œ ๐‘Ž๐‘›๐‘–, ๐‘‘๐‘œ๐‘, ๐‘˜๐‘Ž๐‘ฆ ๐‘›๐‘Ž๐‘Ž๐‘ฆ๐‘œ ๐‘š๐‘Ž๐‘› ๐‘ ๐‘Ž๐‘‘ ๐‘Ž๐‘›๐‘” ๐‘ข๐‘๐‘Ž๐‘›."

She truly believed she had found a safer, more effective alternative to multiple medications. But when I checked her vitals, her blood pressure was 190/100, worse, she already had signs of end-organ damage.

I gently explained to her:
๐Ÿ. ๐‡๐ฒ๐ฉ๐ž๐ซ๐ญ๐ž๐ง๐ฌ๐ข๐จ๐ง ๐ข๐ฌ ๐š ๐ฌ๐ข๐ฅ๐ž๐ง๐ญ ๐ค๐ข๐ฅ๐ฅ๐ž๐ซ. Left uncontrolled, it damages the kidneys, heart, and blood vessels, leading to complications like stroke and heart failure.
๐Ÿ. ๐“๐ก๐ž๐ซ๐ž ๐ข๐ฌ ๐ง๐จ "๐จ๐ง๐ž-๐ฌ๐ข๐ณ๐ž-๐Ÿ๐ข๐ญ๐ฌ-๐š๐ฅ๐ฅ" ๐œ๐ฎ๐ซ๐ž. Chronic diseases require a personalized, evidence-based approach, including proper medications, lifestyle changes, and regular monitoring.
๐Ÿ‘. ๐๐จ๐ญ ๐š๐ฅ๐ฅ "๐ง๐š๐ญ๐ฎ๐ซ๐š๐ฅ" ๐ซ๐ž๐ฆ๐ž๐๐ข๐ž๐ฌ ๐š๐ซ๐ž ๐ฌ๐š๐Ÿ๐ž. Many herbal supplements lack scientific validation and can even worsen certain conditions.

The Philippines is seeing a rise in Chronic Kidney Disease (CKD), with hypertension and diabetes as the leading causes. Many patients, like lola, prefer herbal supplements, believing them to be safe and kidney-friendly. Unfortunately, this belief delays proper treatment, often leading to irreversible kidney damage and, eventually, dialysis.

Here are my take-aways:

๐Ÿ. ๐„๐๐ฎ๐œ๐š๐ญ๐ข๐จ๐ง ๐ข๐ฌ ๐ค๐ž๐ฒ. Patients deserve clear, compassionate, and evidence-based explanations to help them make informed decisions.
๐Ÿ‘. ๐‡๐ž๐š๐ฅ๐ญ๐ก ๐ข๐ฌ ๐š ๐ฃ๐จ๐ฎ๐ซ๐ง๐ž๐ฒ, ๐ง๐จ๐ญ ๐š ๐ฌ๐ก๐จ๐ซ๐ญ๐œ๐ฎ๐ญ. There is no magic pill, but there are proven strategies that can help people live longer and healthier lives.

As she left, I could see hesitation in her eyesโ€”but also a spark of understanding. Hopefully, it was the first step toward real healing.
