MAGANDANG ARAW PO MGA KABARANGAY❗❗❗

Magkakaroon po tayo ng Solo Parents Registration 2023 sa ating Brgy Pungo Covered Court sa darating na Sabado at Linggo January 21- 22, 2023, mula 8AM - 12pm at 2Pm - 5pm.

SOLO-PARENT ID REQUIREMENTS:

*SOLO WITH MINOR (22 yrs old and below)
*Application Form (issued by Brgy. Pres)
*Brgy. Certification (issued by Brgy)
*Photocopy Birth Certificate of Minor Children
*2pcs 2x2 ID Picture (white background)
*1pc 1x1 ID Picture ( white background)

ADDITIONAL REQUIREMENTS if applicable:
* Widower/Widowed (Photocopy Death Certificate)
* Detained Husband/Wife (Latest Detention Certificate atleast one year detained)
*Separated/Annulled (proof that you are legally separated/annulled issued by Brgy, family court, etc..)
* Guardian (Certificate of Guardianship issued by MSWD)
* Bedridden (Latest Medical Certificate and PWD ID)
*Mentally Incapacitated (Medical Certificate)

FOR NON MINORS (NO ID)
*Brgy Certification (issued by Brgy)
