A new study may change the way we think about lip filler injections.

A recent study published in Plastic and Reconstructive Surgery (2026) used ultrasound to examine the actual location of hyaluronic acid fillers after lip augmentation. Surprisingly, in many participants, the filler was found within the orbicularis oris muscle—even though that was never the intended injection plane.

If you inject lips regularly, you've probably experienced this and wondered why.

You feel confident your needle is in a superficial plane, but as you continue injecting, the filler suddenly appears as a translucent lump on the mucosal side of the lip.

So what happened?

Did the filler migrate there on its own, or was it actually deposited there from the beginning?

The study also provides evidence supporting filler migration, highlighting that the final position of filler is influenced by far more than just the injection point.

So, what are the pitfalls?

Potentially... everything.

Needle angle, injection technique, individual anatomy, tissue resistance, filler rheology, and the behaviour of the product after injection may all contribute to where the filler ultimately ends up.

Perhaps there is no single technique that works for every patient.

The real question is:

How can we consistently place filler in the intended anatomical plane? And how can we minimise the risk of migration?

As aesthetic physicians, we are not sharing this story to create fear, but rather to encourage reflection and awareness about patient safety.

Over the past few months, many patients have asked whether we offer dermal filler treatments, especially after seeing that I have completed advanced filler injection training.

The honest answer is: yes, I can perform filler injections.

However, the reason we have chosen not to offer fillers at Skintheory at this time is not because of a lack of training or confidence. It is because filler treatments require much more than the ability to inject beautifully. They also require the expertise, experience, infrastructure, and emergency preparedness needed to manage complications when they occur.

Having spent more than a decade in the aesthetics industry, I have seen firsthand that even highly skilled and experienced practitioners can encounter complications. No procedure is ever completely risk-free.

What truly distinguishes a practitioner is not only the ability to perform the treatment, but also the ability to recognise, diagnose, and manage unexpected complications quickly and effectively.

The truth is, I am fully capable of performing filler treatments, including lower-risk areas such as the lips and chin. However, our current studio was intentionally designed for non-invasive and minimally invasive aesthetic treatments. While it is suitable for the services we currently provide, I do not believe it yet offers the level of infrastructure and clinical support that I personally require before introducing injectable fillers.

Until I am confident that every aspect of patient safety, complication management, and emergency preparedness meets the standards I expect, I would rather wait than compromise.

This case also raises an important question.

If complications can happen even when patients seek treatment from qualified and experienced medical professionals, why are so many people still willing to place their trust in unlicensed injectors or individuals who are not doctors?

Today, you may be fortunate.

Tomorrow, you may not be.

The money saved from choosing a cheaper provider is rarely worth the risk of a complication that could affect your appearance—or your health—for the rest of your life.

Beauty should never come before safety.
