1. Photocopy of Good Moral Certificate
2. Photocopy of PSA Birth Certificate
3. Photocopy of Report Card
4. 2x2 ID Photo (Must have a blue background)

Submission Guidelines:
To ensure a smooth processing of your application, please follow these instructions carefully:

1. Enclose all your required documents inside a LONG BROWN ENVELOPE.

2. Label the front of your envelope with the following details:
-Learner's Reference Number (LRN)
-Full Name (Format: Surname, Given Name, Middle Name)
-Previous School Name

📍 Drop-off Location: Please submit your properly labeled envelopes directly at the RTU Entrance (Wellness Lobby), from Monday to Friday, 8AM-5PM.
