🔮Common Sleep disorders🔮
🛡Nightmares
🔸Nightmares are disturbing dreams that occur during REM sleep, often leading to awakening in distress .
🔹Common between 3 and 5 years of age.
🔸They tend to occur during the last 1/3 or the night.
🔹Muscle tone is inhibited during REM, mobility during the dream does not occur.
🔸Reassure children that the dream is over and they are safe in their bed.
🔹 Talking about it the next day is helpful.
🔸Frequent nightmtares can be due to serious stressors in the household.
🔸 Identify stressors and try to resolve them.
🛡Night Terrors
🔹Night terrors differ from nightmares.
🔸Night terrors produce a distinct non-REM parasomnia.
🔹Most occur within the first 2 hours of sleep.
🔸The child appears to be awake but is unresponsive, difficult to arouse, and unaware of the parent's presence.
🔹Crying can be intense, may appear to be diaphoretic and disoriented.
🔹Dilated pupils, tachycardia, and hyperventilation are usual.
🔸Return to quiet sleep within 5 :20 minutes.
🔹In contrast to nightmares, the child does not remember the episode.
🔸 Occur in 1-3% ofchildren and are more frequent in males.
🔹Night terrors peak at 5 :7 years of age.
🔸Frequently, there is a family history of night terrors.
🔹Reassure parents and advise them to observe the child in episode, without awakening him.
🔸 Awakening a child during night terrors often prolongs the episode.
🔹 A "preemptive awakening" before the usual time of the night terror can break the cycle and prevent episodes.
🔸Up to 1/3 of children with night terrors also experience sleepwalking 👇.
🛡Sleepwalking (Somnambulism)
🔹Sleepwalking is another partial arousal parasomnia.
🔸Child appears to be awake but has a high arousal threshold and is unresponsive to the environment.
🔹Educate parents to ensure a safe environment, observe the child, and gently lead the child back to bed.
🔸As with night terrors, preemptive awakening can be successful in eliminating the behavior.
🛡Sleep Talking (Somniloquy)
🔹Sleep talking can occur during any stage of sleep, from partial arousal to REM sleep.
🔸It is common and may persist throughout adulthood.
🔹No specific intervention is required.
🎯Treatment and Management of Sleep Problems
🔸A regular daily routine is important in the treatment of sleep problems.
🔹Children who are sleep deprived are more likely to have a sleep disorder.
🔸 Place the infant in a crib while drowsy but not yet asleep.
🔹In older infants, the best management of nightwaking is to ignore it.
🔸It is important for older children to have a regular bedtime routine such as bathing, changing clothes, brushing teeth, and reading books.
🔹Younger school-age children need special attention from the parent to hear a story, read together, or talk about activities for the next day.
🔸Avoid using electronic screens near bedtime to minimize difficulties with sleep initiation.
🔹For those afraid of the dark, nightlights are helpful.
🔸When children attempt to delay bedtime by requests advise parents to gently but firmly emphasize, "No, it is bedtime."
