Under RA 9994 (Expanded Senior Citizens Act) and its implementing guidelines, OTC (over-the-counter) medicines are entitled to the 20% discount and VAT exemption even without a doctor's prescription.

Quantity Limits for OTC Medicines

The law allows pharmacies to limit OTC medicines to quantities consistent with personal use:

A pharmacy may dispense only up to a 1-day supply at a time, but

The total quantity sold with senior citizen discount must not exceed a 7-day supply for OTC medicines.

Examples

If the usual dosage is:

Paracetamol 500 mg: 1 tablet every 6 hours (4 tablets/day)

Maximum discounted OTC quantity: 28 tablets (7 days × 4 tablets/day)

Loperamide: 1 capsule after each loose stool, up to 4 capsules/day

Pharmacy may limit the quantity based on a reasonable 7-day supply.

Prescription Medicines

For prescription drugs, the quantity dispensed should follow the physician's prescription and generally should not exceed a 1-month supply in a single dispensing.

Recent Update

As of 2025, senior citizens no longer need to present a purchase booklet to avail of medicine discounts. A valid senior citizen ID (or other acceptable proof of eligibility) and, for prescription medicines, the required prescription are sufficient.

For pharmacists, a common compliance issue is when a senior citizen purchases OTC medicines in quantities far exceeding a 7-day personal-use supply; the pharmacy may lawfully limit the discounted quantity based on the DOH guidelines.
