Siquijor Provincial Science High School announces the conduct of the Preliminary Screening and Selection Process for incoming Grades 7 and 11 Learners SY 2025-2026 on April 9, 10, 11 and 12, 2025.

The school will start accepting the needed requirements from interested learner applicants on March 24, 2025 to April 7, 2025.

Requirements:
(All must be placed in one long folder)
*Application letter signed by the parent/guardian
*Duly accomplished Learner’s Profile Form
*Birth Certificate from PSA/LCR
*Learner’s Progress Report Card with a grade average of 87% for English, Math and Science and no grade average below 85% for other subjects from first to third quarters in Grade 6 (for incoming Grade 7) and in Grade 10 (for incoming Grade 11)
*Certificate of Good Moral Character
*Certificates of Academic and Non-academic Awards (arranged from the latest)
*Duly accomplished Declaration of Medical Condition Form
