You cut the carbs at dinner — but you still wake up with high blood sugar. That’s not your dinner; it’s your liver. Your liver is your blood-sugar thermostat, but when it gets clogged with fat — a fatty liver — it turns insulin resistant and leaks glucose into your blood all night, so you wake up high even though you ate nothing. How does it get fatty? Not from fatty food — from sugar and refined carbs, which your liver turns straight into fat. The honest part: that fat is stubborn and comes down slowly — but your liver’s FUNCTION improves faster, so your morning blood sugar comes down long before the fat clears. That’s your first sign it’s working.

Your diabetes medication keeps going up — but for type 2, the problem was never too little insulin. You’re making plenty, sometimes too much; your body has just stopped listening to it. Two different things get mixed up: how much insulin your pancreas MAKES (production) versus how well your cells RESPOND to it (resistance). Type 2 is a listening problem — which is why adding more insulin just shouts louder and the doses keep climbing. The real work is getting your cells to respond to the insulin you already make. Willie proved it: 30 years diabetic, insulin up to 18 units a day — and once his cells started responding, he came off it completely.

You can be fit and still be diabetic. Franklin was 57, lean and muscular — body fat under 19%, metabolic age 32 — and still pre-diabetic. Because insulin resistance isn’t about how you look; it’s about how your cells handle sugar, and a lean body hides it. The number that revealed it was his triglyceride-to-HDL ratio: 2.5, deep in insulin-resistant territory. We corrected the resistance underneath — in under three months his ratio fell to 1.4, his triglycerides dropped 37%, and his body fat came down from 19 to 16%. Check yours: under 0.7 (mmol/L) or under 1.5 (mg/dL) is healthy.

You’re eating “healthy” but your blood sugar is still high — and you can’t tell which food is doing it. A continuous glucose monitor (CGM), a small sensor on your arm, shows you the spike after every meal — so you stop guessing and start seeing. The foods you trust (wholemeal bread, oats, fruit juice) may spike you hardest, because insulin-resistant muscle can’t clear the glucose. In this video: how to use one for two weeks, find your worst meals, and test the fixes (protein/veg first, a post-meal walk). Remember — a monitor shows the problem; fixing the resistance is the work.

You’ve cut the carbs and done everything right — and your blood sugar still won’t come down. Here’s the piece nobody told you: your muscle is the biggest place your body clears glucose after a meal (~80% of it). When your muscle is insulin resistant, that glucose has nowhere to go, so your sugar stays high no matter how little you eat — that’s why diet alone isn’t enough. A short post-meal walk lowers that spike, but walking alone won’t rebuild the muscle — two short strength sessions a week do more than 10,000 steps. It was never your willpower. It’s your muscle.

Every year your blood sugar creeps a little higher — and your doctor adds a little more medication. That’s not you failing. Early on, your pancreas has plenty of reserve: when your cells resist insulin, it just pumps out more, keeping your sugar looking okay for years. But those insulin-making cells can’t overproduce forever — they tire, your sugar climbs, and the meds increase. It’s not your discipline; it’s your pancreas losing reserve. The good news: those cells are tired, not dead — ease the resistance early and they can recover. And even years in, it’s not too late (Willie: 83, 30 years diabetic, 18 units of insulin → zero).

You switched to basmati because it’s “low GI” and “better for diabetics.” But GI is measured on a fixed lab portion, in healthy people — not your bowl. The number that actually matters is glycemic LOAD: the GI multiplied by the carbs on your plate. A real bowl of basmati still carries a high load, and the “low-GI” label just gives you permission to eat more. In this video: why the label misleads, and the simple change that beats hunting for special rice.

If you snore, or wake up tired no matter how long you sleep, your blood sugar could be getting worse while you sleep. Heavy snoring is often sleep apnea — your airway collapses and your oxygen drops all night, which triggers stress hormones that push your blood sugar up and deepen insulin resistance. And the insulin resistance stores the very neck fat that makes you snore. In this video I show the two-way link — and simple changes that help tonight.

If you get gout, you’ve probably been told to cut seafood, beer — and even tofu and beans. But plant purines from tofu and lentils don’t raise your gout risk. The real drivers are fructose and insulin resistance: sweet drinks turn straight into uric acid, and high insulin makes your kidneys hold on to it. In this video I bust the tofu myth, show what actually raises uric acid (Dr David Perlmutter’s “Drop Acid”), and the first change that helps.

Your HbA1c can read “fine” for years while insulin resistance quietly builds underneath. The test that catches it early — fasting insulin, or HOMA-IR — isn’t on the standard panel, and almost no doctor orders it. But there’s a shortcut already on your last blood report: divide your triglyceride by your HDL. Under 0.7 in mmol/L (or under 1.5 in mg/dL) is healthy — the higher above that, the more insulin resistant you already are. In this video I show you how to read it — and how Elaine took her ratio from 1.93 to 1.2.
