Atresia of biliary tract type || A
2 months old male was referred to our pediatric surgery clinic with a biliary tree scintigraphy and liver biopsy diagnosing biliary atresia.
The patient was admitted in our pediatric surgery department and surgical procedure was performed the day after the admission .
In this case we want to point that there are many surgeons when they perform biliary atresia surgery ( kasai procedure or the porto-enterostomy ) tend to mobilize and fully expose the liver through a Chevron incision , so they can take more control in the porta hepatis , isolating the right and left portal vein and hepatic artery , in our institution we believe that this technique is too crucial has a potential invasiveness , and enough exposing for kasai porto-enterostomy without it , by performing a right subcostal incision , a Deaver retractor is used , to expose the inferior surface of liver , the gall bladder is dissected from its bed , on table cholangiography is performed , then the superficial peritoneum in the hepato-doudenal ligament is opened and the anatomy of the bile duct, the hepatic artery , the portal vein is assessed , in our case the atresia was at the proximal level of the Common Hepatic Duct extends to the porta hepatis , so the atretic part of the CHD was dissected till we reached the porta , with the respect to the portal vein and hepatic artery which were dissected carefully from the CHD
The CHD was excised , then the kasai procedure was performed as in the following pictures .
After many operations for biliary tract malformations,We believe that this technique is better because it is less invasive , and the control of the bile ducts could be done with the same results , exposing the liver through a Chevron incision should be done while performing liver transplantation, , or when there is a traumatic injury on the liver and more investigation of the portal triad , the inferior surface of the liver parenchyma , and if there is an area of the parenchyma can’t be reached without exposing the full liver like in tumors …
