A New Model Emerges for Amputee Care | Amplitude Doctors and prosthetists are shifting toward a more holistic amputee care model, one that promises to deliver a better experience and better health outcomes for patients.

⭐️TMR Tuesday⭐️

It’s been shown that following a TMR procedure, nerve pain can increase in the first few weeks secondary to the nerves being “excited” as they grow into their new targets.

To combat this, we use various modalities to help during this period, including the use of medications, massage, PT/OT techniques and an OnQ pump that drips numbing medication around the nerve for about a week following surgery. As we are always thinking outside the box and finding ways to better help our patients, we have started to use a peripheral nerve stimulator, which is implanted during surgery directly next to the nerve, and is used in the post-surgical period to further help in pain control. The patient controls the StimRouter themselves via a remote with a multitude of programs and intensities which are specifically tailored to help with their distinct pain patterns.

Dave, who has suffered from tremendous pain in his right hand for years following a traumatic accident, is one of the first patients we have placed the StimRouter in. Not only has his pain been substantially decreased in his post-operative recovery but he also has been able to reduce his use of pain medication which he has been on for years. We are so excited to see the positive results and are continuing to study how this StimRouter can best be used to help our patients- both in limb loss and limb salvage.

We want to thank the Hartford Hospital Bone and Joint Institute for asking us to present grand rounds this morning via zoom. We discussed all things ICAN, including how we’ve grown our multidisciplinary clinic, what we have found to be beneficial for our patients, and the various techniques we use in both amputation/nerve repair and limb salvage. It was such a pleasure meeting with everyone and hope our vision will help lay the groundwork for Hartford Hospital’s own multidisciplinary clinic in the near future!
.... and in true ICAN fashion, we all immediately proceeded to the OR to tackle a complex limb salvage case together 💪🏻

Huge thanks to Dr. Crandell at Spaulding Rehab for referring me to the ICAN Team at Mass General- Dr. Eberlin, Dr. Heng, Dr. Valerio, Jenna and Jayne Drummey at Hanger. They did TMR surgery and revised my leg for a better fit. For the first time, I am now walking pain free. So grateful to everyone on the team for taking the best care of me!”

“I’m three weeks post reconstructive surgery and TMR on my residual limb. So far, so good. This team has been awesome to work with! The whole team has been present for each appointment which is a big plus! I get to see all the specialists in one appointment and they all collaborate right there on the spot with me. ⭐️TMR Tuesday⭐️

So if you ever wondered what was inside of you, causing all that “shooting/buzzing/zinging” pain? Well, that ball of scarred nerve is the main culprit.

▪️Neuromas come in all different sizes and shapes and even a tiny neuroma may cause a significant amount of pain.
▪️The neuroma is what is removed during TMR surgery and the healthy portion of the nerve is then re-routed nearby
▪️It has been shown that pain is significantly improved in TMR patients as compared to people who undergo traditional amputations.
▪️ Here at MGH, TMR is the standard of care for almost all of our acute amputations, as it has minimal health risks and offers our patients a greater chance of living a life with little or no pain.
▪️ TMR can also be performed years after an initial amputation to help reduce an individual’s pain.
