The duties of an HTS include: cooking meals, light house work, assistance with hygiene, community activity planning, medication administration, goal setting and achievement, independence skills as well as daily charting.

Personal Care Aide

Job Duties
• Maintain confidentiality of all patient information.
• Complete meal preparation assistance to include preparing meal, serving meal, cleaning up meal preparation area, and assisting your patient with eating as needed and applicable.
• Complete personal care assistance to include tub, shower, or bed baths, assist with dressing, grooming, denture care, and skin care.
• Complete housekeeping tasks to include sweeping, mopping, vacumming, taking out trash, dusting, cleaning out the refridgerator, and washing/putting away dishes.
• Complete laundry tasks to include washing, drying, folding, and putting away of all laundry. Performing laundry tasks at a laundromat if needed.
• Assist with running errands to include grocery shopping, picking up medical supplies, running to the post office, etc.
• Rarely absent from work and is reliable, dependable, and shows up on time.
Job Requirements

Job Descriptions:
• obtain medical history from patients
• Develop and implement an individualized plan of care on all assigned patients.
• Supervise home health aides on a routine basis to accomplish the plan of care. Performs and submits home health aide supervisory notes per agency policy.
• Comprehensive Initial Assessment, SOC/IDT
• Recertification assessments
• Reports pertinent observations and reactions regarding changes in patients status on the appropriate team person and provides follow-up to assure resolution of any identified problem.
• Participate in weekly meetings to ensure optimal care is being provided.
• Maintains acceptable standards of practice as evidenced by quality assurance activities.
• Records observations accurately and concisely.
• maintains and completes patients medical record in accordance with agency policy.
• Provide accurate, timely, complete, and acceptable documentation.
• Work with case managers to initiate care plan and make changes.
