Leaking tends to get all the attention, but you can have pelvic floor dysfunction without having leaking issues.
Pelvic floor dysfunction is rarely one symptom. It’s a group of muscles that has stopped coordinating, and it shows up in whatever it happens to affect first. Urgency is often a bladder reacting to a floor that never fully lets go, so it signals full long before it is. Heaviness or pressure is a support issue, not a sign that something is about to fall out. And pain that tracks your cycle means hormone shifts are landing on tissue that’s already working too hard. All of it is treatable.
August is Pelvic Floor Dysfunction Awareness Month, which is as good a reason as any to stop filing this under normal. At our clinic in Williamsburg, Brooklyn, an appointment is an 90 minutes of hands-on work: a full assessment, manual therapy for the muscles that won’t release, and retraining the coordination piece that kegels alone were never going to fix.

If you're postpartum and you're still not finding yourself "in the mood", you are not alone or broken or doomed. Desire to be intimate after having a baby doesn't just bounce back on it's own. Your tissue is still healing and a pelvic floor that pushed out a baby or braced through a c-section, is often weakened, gripping, or both. Breastfeeding keeps estrogen low, which means dryness and burning that you have no control over. And a nervous system running on four hours of sleep is not going to prioritize arousal, it's going to prioritize surviving. It's all normal, and it's all things you should be aware of and should discuss with your partner if that's something you're worried about.
Your partner isn't in your body, so sometimes having gentle conversations like these can help them understand where you're at.

Somewhere along the way, a lot of women were handed the same BS advice about pain with s*x: use more l**e, have a glass of wine, and get through it. That’s a huge NO from us.
Painful s*x usually has a physical, findable cause. It might be a pelvic floor that’s holding tension it never released. It might be scar tissue from birth or surgery, or a nervous system that’s guarding. It doesn’t get better by pushing through or “sucking it up”, but it does get better with treatment.

Twenty minutes of exercises, three times a day……no thanks! But that’s the sheet that most people walk out of pelvic floor therapy holding. By week two it’s under a pile of mail and then you’re feeling guilty about not doing your exercises, and about the pile of mail.
Here’s what most people are never told: if your nervous system is stuck in fight or flight, no exercise protocol will hold. When your body thinks it’s being chased by a bear, it doesn’t prioritize digestion, sleep, pleasure, or a pelvic floor that knows how to let go. You can’t strengthen your way out of a muscle that won’t stop guarding.

You don’t need to apologize or feel weird about a single thing on this list. And one thing about us? We’re going to keep reminding you of that until you believe it.
Queefing during Pilates…air gets in during bridges and then air comes back out. Every instructor here has heard it a thousand times and could not care less. It’s a good sign that your pelvic is reacting to the movement!
Your libido changing through the month…desire rises and falls with your hormones. High around ovulation and lower before your period is textbook. You’re not broken in week four, you’re just in week four.
Not working out every day…if you’re exhausted, overworked, sick, need to rest, or just aren’t feeling it, that’s okay. Muscles rebuild on your days off.
Talking to your pelvic floor therapist about s*x…we encourage this!! Nothing will shock us. People tend to whisper this in the last five mins of a session, but we encourage you to bring it up in the beginning. Questions, observations, curiosity, it’s all welcome and helps us direct your treatment.
Crying on the massage table or mid pelvic floor session…the body stores a lot of stress and trauma in the hips and pelvis, and release can bring up a lot of emotions. We’re here to guide you through that. You never, ever have to apologize for any emotions that come up during sessions.
Not orgasming with pe*******on alone…most women don’t! And that’s due to anatomy. Nothing is wrong with you if it takes more stimulation for you.
Period poops…the same chemical signals that make your uterus contract get your bowels moving too. Totally, completely normal.

Neither is crossing your legs before you sneeze or deciding s*x just feels different now.
If you’re leaking, feeling pelvic heaviness, or hurting during exercise past 3 months postpartum, your recovery needs more than time. And if you have none of those symptoms? You can still benefit from pelvic floor therapy postpartum.
Pregnancy changes your pelvic floor no matter how you delivered or how smooth your recovery feels. In France, pelvic floor therapy is standard postpartum care. Here, it’s more common to get a six week checkup and a wave goodbye. “Feeling fine” and “fully recovered core and pelvic floor” are not the same thing.

That guarding can be unlearned through trauma-informed pelvic floor therapy. Nervous system down-regulation, gentle manual therapy and scar tissue work, and breath-led release can help get you there.
If your birth story still lives in your body, you’re not being dramatic and you’re not stuck with it.
