Got called back from your screening mammogram? Thanks to a new federal law earlier this year, your additional imaging is likely going to be free to you — no out-of-pocket cost. Here’s the thing: almost nobody knows about this, including most radiologists.
As of January 1, 2026, a federal rule now requires ACA-eligible plans — which is most plans out there — to cover that callback with zero cost-sharing. Extra mammogram views, ultrasound, even biopsy and pathology, as long as it’s directly linked to your screening mammogram.
Why does this make sense? As a fellowship-trained breast radiologist, a screening mammogram only ever gets one of two calls from me: normal/benign, or incomplete. There’s no such thing as “abnormal” at the screening stage. A callback isn’t a new, separate problem — it’s a continuation of the same exam, working toward a definitive answer. That’s exactly why the coverage was extended to follow it through.
Here’s the catch: this only works if your radiologist’s report clearly documents that the additional imaging or biopsy is linked back to the original screening finding. That documentation is what allows billing teams to apply a modifier (called a 33 modifier) that signals insurance to cover it. Without it, that cost can get passed to you instead — and nine months into this rule, a lot of facilities still aren’t doing this consistently.
At GRACE, we’ve built this into how we report and bill, because we don’t think a bill should be the reason a woman doesn’t get her definitive answer. If you’ve been avoiding scheduling a screening mammogram because of a past callback bill, it may be worth revisiting.

Two patients. Two moments where expertise changed everything.
One biopsy showed inflammation — but one question from our fellowship-trained pathologist, paired with a key patient detail, uncovered a rare diagnosis. Instead of returning to routine screening, that patient was referred straight to surgery.
Another patient had already seen multiple general radiologists before coming to us. This was her third biopsy of an area that looked worrisome on imaging — and each time, pathology called it benign. Something didn’t add up. We trusted the imaging and sent it out for a second pathology opinion. It came back grade 2 invasive cancer.

📞 Got “the call” after your mammogram? Take a breath — here’s what happens next.
If you got a callback for additional imaging after your screening mammogram, you are not alone — and most of the time, it turns out to be nothing. A callback usually just means your screening came back incomplete, and your radiologist wants a closer look at a specific area before calling it done.
At GRACE, we get you in quickly for diagnostic workup — often extra mammogram views or a targeted ultrasound. Every patient gets their results directly from a fellowship-trained breast radiologist before they leave — no waiting days for a phone call or portal message.
💰 Good news on cost: Under the updated HRSA Women’s Preventive Services Initiative guidance, for ACA-compliant, non-grandfathered plans with plan years starting in 2026, follow-up diagnostic imaging after an incomplete screening mammogram is covered with no out-of-pocket cost. (Medicare and grandfathered plans are handled differently, so we always recommend double-checking your specific plan.)
