New Patients:
At this time, my books and schedule are full, so I am not taking any new patients.

Refill:
All refill requests require the refill form to be filled out, followed by an appointment to discuss and answer any questions.

Licensure:
I am licensed in Nebraska, Iowa, and South Dakota only so I only take patients that reside in these states.

Working Hours:
Please message/text/call only during business hours so I may spend time with my family. Please allow me 24-48 business hours to respond.
