There’s an important difference between a client asking for more pressure and a session actually becoming deep tissue massage.
Pressure alone does not define deep tissue. A client can ask for firmer pressure during a Swedish massage without suddenly receiving a completely different service. Deep tissue is more about the intent, technique, tissue depth, specificity, pace, and treatment goal. If I slow down, assess a problem area, work specifically into deeper structures, change techniques, and start addressing dysfunction rather than simply increasing pressure, now we are talking about a change in the type of work being performed.
This is also why I think massage pricing deserves a larger conversation. If you have a big toolbox and are comfortable moving between Swedish techniques, deep tissue, trigger point work, injury treatment, stretching, myofascial work, and whatever else falls within your scope and training, one of the easiest and most client-friendly models is simply charging by **time**: 60 minutes costs X, 90 minutes costs X, and you use the techniques that person needs that day. The client does not have to diagnose themselves before booking an appointment, and you are not trying to decide halfway through the massage whether you have crossed an invisible $20 line.
Because really, how many clients know the difference?
Someone may book Swedish because they think “deep tissue” means being beaten up for an hour. Maybe their last deep tissue massage left them bruised and sore for three days. They may walk in saying, “I don’t want deep tissue, but I have this horrible crick in my neck. Can you please fix it?” Meanwhile, what they actually need may be focused therapeutic work.
That is where your intake conversation matters.
If you work in a spa that charges one price for Swedish and another for deep tissue, you need to understand exactly what the spa considers the difference between those services. More importantly, think carefully about your own skill set and what you are actually being asked to provide.
If someone booked Swedish because they didn’t know what to choose, but during intake they tell you their primary goal is getting relief from a specific painful restriction, I would clarify that before the massage begins. Explain what you recommend, what the treatment will involve, whether it changes the service they booked, and whether there is a price difference. That is much cleaner than waiting until 40 minutes into the massage and telling them the pressure they like costs more.
There is also an ethical piece to this. If a client is on your table and says, “Can you help me with this knot in my neck?” I don’t think our first thought should be, “You only paid for Swedish.” Our first thought should be, “What does this person need, what am I qualified to provide, and what did we agree to during intake?”
That does not mean giving away an entirely different service for free. Spas are businesses, therapists deserve to be paid for their expertise, and there can absolutely be legitimate reasons for different service prices. But if the only meaningful distinction between an $80 Swedish massage and a $100 deep tissue massage is how hard the therapist pushes, I think the service menu itself needs some rethinking.
And please remember: firmer does not automatically mean deeper, and deeper does not automatically mean better. Deep tissue should never mean “bruise and abuse.” Effective therapeutic massage is about clinical reasoning and appropriate technique, not seeing how much pressure the client can survive.
A good intake solves a lot of this before anyone ever gets on the table.
