Maxie is not a doctor or veterinarian and can not diagnose, prescribe medications, or cure you or your animal. If you or your animal seems to be injured please see your health care professional or veterinarian immediately. A general release and medical history form will be required to be completed prior to the first service for humans. By Texas law, therapeutic massage on an animal is required to be done under the direct or general supervision of a licensed veterinarian. Since Maxie is not a veterinarian and is acting as an independent sole proprietor, a veterinary supervision form is required prior to your dogs first massage in order to be in compliance with Texas Administrative Code §573.14. All files are kept confidential and only released with your permission and signed medical release form. Florida does not require this form. As of now services are by appointment only.
