🌿 WHY PROPER REHABILITATION MATTERS — AND WHY IT SHOULD BE INDIVIDUALIZED

Let me share two clinical scenarios that demonstrate why individualized rehabilitation matters.
🦵 SCENARIO 1: AN OLD KNEE INJURY — YEARS LATER, SHOULDER AND NECK PAIN
A patient had a history of Osgood-Schlatter disease of the knee following an injury at 10 years old. Fifteen years later, the patient developed shoulder and neck pain that affected sports activities.
The patient demonstrated a pattern of reduced weight acceptance through the painful knee, limited knee mobility, weakness throughout the lower extremity and pelvic stabilizers, and compensatory changes in pelvic and trunk alignment.
These movement adaptations were associated with:
🔹 Altered pelvic positioning
🔹 Trunk side-bending
🔹 Changes in spinal curvature and thoracic rotation
🔹 Shoulder blade positioning changes
🔹 Compensatory head and neck positioning
The patient’s complaint? Shoulder pain.
The important lesson: An area of pain may be influenced by movement impairments elsewhere in the body.
💪 SCENARIO 2: AN OLD HAMSTRING INJURY — NOW LOW BACK, SI AND SHOULDER PAIN
A patient had an old hamstring pull injury that was never properly rehabilitated.
Over time, the patient developed altered weight acceptance through the injured leg, lower-extremity weakness, and changes in foot and pelvic mechanics.
The patient also demonstrated compensatory changes in spinal alignment and trunk positioning.
Current complaints included:
🔹 Low back pain
🔹 Bilateral SI joint region pain
🔹 Right shoulder pain
The original injury was in the hamstring — but the symptoms were no longer limited to the hamstring.
⚠️ WHY DOES THIS HAPPEN?
The body is constantly trying to find a way to move, function, and protect itself.
When an area is painful, weak, restricted, or poorly controlled, the body may develop alternative movement strategies.
Over time, these strategies may become habitual and contribute to:
• Muscle imbalances
• Limited joint mobility
• Altered weight distribution
• Poor postural control
• Compensatory movement patterns
• Pain in other regions
However, every person compensates differently. There is no single pattern that applies to everyone.

At Netherine’s Physical Therapy, we believe scoliosis care should go beyond simply strengthening the muscles around a curved spine.

When “Tightness” Is More Than a Muscle Problem
