Six months following rhinoplasty, this patient is healing beautifully and already showing a wonderful result.
One of her primary goals was to address the prominence of the nasal bridge.
At this stage, the profile is smoother and more balanced, creating a straighter contour that fits naturally with her other facial features. The bridge has been refined, the tip is more defined, and the overall shape of the nose feels harmonious with the rest of her face.
What is important to remember is that healing after rhinoplasty continues well beyond the first few months. While she already looks excellent, subtle refinements will continue as the remaining swelling resolves, particularly in the nasal tip.
This is one of the reasons patience is so important after rhinoplasty.

There's no single “right” age for facial rejuvenation, and the answer is very individual. Two people the same age can be in completely different places, and that's exactly why I never go by a number.
What actually matters is your anatomy: how your skin is aging, the quality and elasticity of your tissues, the degree of laxity along your jawline and neck, and your bone structure underneath.
I've seen patients in their 40s who are ideal candidates because they want to address early changes before they progress, and patients well into their sixties and seventies who get beautiful, natural results because their tissue quality is excellent.

This patient is one year following a deep plane facelift and deep neck lift.
When people think about a neck lift, they often picture tightening loose skin, but the structures responsible for a heavy or undefined neck usually sit much deeper.
Surface work can temporarily smooth things, yet it does not change the underlying anatomy, which is why those results often soften within a year or two.
My approach focuses on those deeper layers. After elevating the platysma, I address what lies beneath it: the subplatysmal fat, the digastric muscles, and, when appropriate, the submandibular glands.
These are the structures that blunt the angle between the jaw and the neck. Refining them is what creates a sharp, defined contour rather than a neck that simply looks pulled.
The platysma is then reapproximated in the midline, so the muscle supports the new shape from within rather than relying on tension at the skin.
The deep plane facelift follows the same principle in the face. The deeper layer is released and repositioned as a unit, and the skin is redraped over that foundation with no tension. This is why the result develops a “natural look” that people love to see.
One year out, all of the swelling has resolved and the tissues have fully settled, so what you see is the true, stable result.

“Can one side be better than the other?”
It is a reasonable question, but in my experience, significant side-to-side differences are not something I commonly see.
During fat grafting, the fat is carefully purified and placed in a smooth, layered pattern from multiple directions. Once transferred, the surrounding tissues develop a blood supply to support the grafted fat. When meticulous technique is used, the process is remarkably predictable.
There are factors that can influence how much of the transferred fat survives, including age, overall health, and smoking history. However, those factors generally affect the face as a whole rather than causing one side to behave differently from the other.
Typically, about two-thirds of the transferred fat establishes a blood supply and remains long term, while the remainder is naturally absorbed by the body over the first several weeks of healing.
What I appreciate most about fat grafting is how natural the results can be. When performed thoughtfully, it is an excellent way to restore volume and create a refreshed, balanced appearance.

Two and a half months following a rhinoplasty, and she is already looking fantastic!
These are still early results, with more refinement to come as swelling continues to settle, especially in the tip.

- Reduce the dorsal hump
- Rotate the nasal tip to correct tip ptosis
- Correct the groove of the nasal tip
- Refine and support the nasal tip

Do you ever wonder why some people develop under-eye bags more than others, no matter how well they take care of themselves?

One question I occasionally get from patients is whether I use endotines during brow lift surgery.
Endotines are small, dissolvable implants used to anchor the brows after they have been repositioned. They are effective, but because they can sometimes be felt through the scalp while they dissolve, many patients prefer other options.
What I think is more important than the specific anchoring method is how the brow is released before it is lifted.
A key part of the procedure is releasing the arcus marginalis, one of the structures that help hold the brow in its descended position. When the brow is fully released, it can be repositioned without significant tension.
At that point, the anchoring technique simply needs to hold the brow in place while healing occurs over the next several weeks.
As with many aspects of facial plastic surgery, the discussion often focuses on the tools being used. In reality, the success and longevity of the result usually depend more on the surgical technique and thoughtful ex*****on than on any single device.
