All proofs of residency must be printed out, and all government-issued documents (birth certificate, passport, marriage certificate, etc.) must be original — no copies accepted.

If your name is different from the name on your birth certificate, you must provide legal name change documentation, as well as marriage certificates and divorce decrees if applicable.

In the link below, you will find the sign-up sheet for applicants and a link to upload documents.

Note: There is an option to upload documents for preapproval within one week before the event.

You must still bring all documents to the event for final approval.

Upload Instructions:
Upload your documents no sooner than 30 days prior to the event.

To guarantee preapproval, all documents must be uploaded at least 5 days before the event.

Fees:
Duplicates: $8.00 for the first, $12.00 for each additional duplicate

Renewals: $28.00 and up, depending on endorsements

ID Cards: Under 18 — $4.50; Over 18 — $12.00

All new ID cards require the documents listed above

Reinstatement Information: Free letter with detailed requirements (must be printed)
