I’m a private nurse care manager. This may be your first time navigating all of this. I do it every day.

I’ve been a Registered Nurse for 13 years, and I’ve been doing in-home care since 1997.

Here’s what that actually means:

I can attend appointments with your mom, ask questions, make sure her concerns are heard, take her home and update her medication box with the new changes.

I can also fill medication boxes every two weeks or monthly. From a simple pill organizer to a locked and safe dispenser with timers, alarms or missed-dose notifications, I help find a setup that works for your family.

I help during hospitalizations, when you’re trying to understand the plan, ask questions and make sure your loved one’s needs are heard.

I help navigate a safe discharge plan, including medications, equipment, follow-up appointments, transportation and the support needed at home. When the hospital or care facility feels overwhelming, I help you understand what’s happening and what comes next.

I find free or low-cost community resources, including rides, so family members don’t have to do all the driving. I help you access insurance-covered benefits you may not even know are available. I can help ensure the open enrollment period sets next year up for success.

I help your dad understand the pros and cons of moving into assisted living. And I help your family understand what IS and IS NOT included before signing the paperwork.

“Senior living” can mean an apartment and three meals a day. It doesn’t automatically mean help with medications, bathing or getting to appointments. Having a nurse on-site doesn’t necessarily mean that nurse manages your loved one’s individual care. We ask those questions before the move.

There’s a good chance I’ve been to the facility you’re considering or am familiar with the doctor or specialist you need. I bring years of experience helping people find their way through these decisions.

I follow up on referrals and appointments that get lost in someone’s to-do list or never-ending inbox.

I help you understand a new diagnosis, the road ahead and the questions to ask about your options. That includes seeking a second opinion and making sure the plan reflects what matters to YOU. I have more time than the quick visit your insurance covered, let’s maximize that.

I explain the difference between palliative care and hospice. They are NOT the same.

I help families understand what support is available through benefits, insurance, community programs and private-pay services.

For families who live far away, I can be the local eyes and ears. I assess what’s working, what’s becoming harder and what could stand in the way of living independently.

I help families explore technology, home safety tools, resources and referrals that support aging in place.

Because the goal isn’t simply to get through the next appointment.

It’s to help people spend their days in their preferred place, with the support they need and a voice in their own care.
