One is a Covid waiver form and the other is a Credit Card authorization form. Both need to be signed before your appointment will be accepted. They are both required. Please note: Appointments will be cancelled if not confirmed, rescheduled or cancelled 24 hrs prior to your appointment. Please select the links for confirm, or reschedule/cancel. Please read my cancellation/No-show policy either on my INSTAGRAM or my website @ Pamperhertoosugaring.com (http://www.Pamperhertoosugaring.com) . If you are more then 10 minutes late there will be a $10 charge. If your are more then 15 minutes late, I may not be able to see you and your deposit will be forfeited and a new deposit is needed to re-book. Also, A credit card is now required to book an appointment. If appointment is booked but credit card was not saved, the appointment will be cancelled. Please be sure to save a valid card on file. Also, full payment is now required for appointments longer then an hour. Thank you all for your patience. I apologize I have to now set these rules due to too many cancellations, & no-shows.
