I will be asking everyone new and old to fill out a new intake form.

Intake forms are very important to help the therapist understand the contraindications that might hinder your progress with your massage.

I’m happy to say that I have plenty ready for you to come by the office anytime this week to get it before your visit, I can email you a copy, or please plan to come a little early or plan to stay a little late the day of your next massage.

Also, this will help providing me with information of addresses and birth dates. These will become very important as the year progresses for you as a client and me.

Looking to book with me- I have sometime available. As a reminder I can only do so many clients a day. If it looks like a lot of availability one day it may not actually be as much as a variety of times and I can only take one or two clients that day.
