Under eye filler – placed on the orbital bone/deep fat pads. 👀
Filler has gotten a bad reputation… but the real issue isn’t the product (well not entirely at least 😏), it’s where and how it’s being used.
Hyaluronic acid filler is a powerful tool when used with intention – in low volumes and within the correct anatomical compartments to restore natural volume loss that occurs with aging.
When placed deep along the orbital rim and within deep fat pads, filler integrates, supports, and behaves predictably.
Migration risk is significantly reduced.
But in my experience, especially in the tear trough, filler is often placed too superficially – within the muscle.👇🏼
Second slide: histopathology from one of my favorite articles by Dr. Mraz Robinson et al on filler migration – something I often reference with patients and in my talks. It demonstrates filler within muscle, with adjacent tissue changes and abnormal extracellular matrix deposition.
This is where problems begin.
Filler in the wrong plane can:
– Disrupt normal tissue architecture
– Trigger inflammation
– Expand over time
– Create progressive fullness that wasn’t intended
This is the cycle I see often:
Old filler expands → more filler is added to “correct” it → the area becomes increasingly overfilled.
This is the cycle I break in many of my patients.
The challenge is that deep orbital injection requires precision and anatomical mastery. Accessing this plane is difficult and it demands a true understanding of orbital anatomy (who better than an oculoplastic surgeon 😉), but it’s the correct one.
Filler isn’t bad.
It’s been misplaced.
My approach:
– Deep, structural placement
– Respect for orbital anatomy
– Conservative volumes
– Long-term thinking

👁️ Upper eyelid blepharoplasty 👁️
In my younger patients, the goal is not to change identity - it’s to refine it.
This means conservative skin removal paired with precise fat sculpting to restore a clean, natural upper eyelid-brow relationship.
Aggressive fat removal is one of the biggest mistakes in eyelid surgery. The upper orbit continues to lose bony support with age, and over-resection accelerates hollowing and skeletonization over time.
Instead, I preserve and reposition fat along the orbital rim, creating a soft structural buffer that supports the eye as the face evolves.
Surgery isn’t just about what you see today. It’s about understanding how the eyelid and orbit will age - and operating accordingly.
This is how we create results that look natural now and remain beautiful long-term.

Never did I imagine any of this. We came to the US from Iran when I was 2 with nothing. We had lost so much and had left everything behind. All because of the war. I’m from a town in Iran called Ahvaz, one of the most heavily affected by the war. My uncle was struck by a missile when he was 20 and died in his mother’s (my grandmother’s) arms. My brother learned how to walk during a missile strike when my family was running for shelter. We lost so many loved ones, our home, everything my family had worked so hard to build. What my family experienced isn’t something any human should endure.
Everything I have and everything I am is because of my family’s utter devotion to me and my dreams. War is a pure dark destructive force and yet my family still chose love. They chose love when the world had shown them so much hate, so much destruction. That is what they instilled in me. The active choice to love. 👁️ Upper eyelid blepharoplasty 👁️
In this young, beautiful patient, the goal was refinement - to create a natural yet powerful result.
A conservative amount of excess skin was removed, while fat was carefully sculpted rather than aggressively excised.
What matters most in younger patients is protection of their aging process.
As the orbital rim resorbs over time, volume loss and hollowing develop along the upper eyelid. Over-resection of fat early on can accelerate this.
For this reason, I approach fat in a forward-thinking way.
Rather than removing it, I strategically sculpt and preserve volume along the orbital rim to support the eye long term.

Lower eyelid blepharoplasty with fat transposition, conservative skin pinch, and CO2 resurfacing.
Fat was repositioned – not removed – to smooth the lid–cheek junction.
Excess skin was conservatively excised.
Fractionated CO2 resurfacing refined texture and fine lines.
Goal: a smooth, natural transition between the lower eyelid and cheek without hollowing or altering eye shape. 👁️
