Schedule: Primarily Monday-Friday, approx. 8 a.m. - 4:30 p.m.

JOB SUMMARY: The RN Case Manager is responsible for monitoring the health and safety of Members enrolled in the My Choice Family Care Managed Care Organization (MCFC MCO). Primary responsibilities include: conducting timely assessments, identifying individualized Member outcomes, completing RN comprehensive assessments and Member centered plans (MCPs), providing ongoing monitoring of health and functional status, providing assistance with accessing acute/primary care and other medical services, providing education about disease management, wellness and prevention, and mitigating risk.

Open to RNs with Associates Degree or BSN.
