Grade 1 (Mild): Acne simplex characterized by blackheads or whiteheads.

Grade 4 (Severe): Acne vulgaris characterized by deeper cysts (large, painful, pus-filled bumps) and nodular lesions (a severe form of inflammatory acne that creates firm, painful lumps), blackheads, whiteheads, papules, and pustules.
