JULIUS WAGNER-JAUREGG: THE DOCTOR WHO USED FEVER TO TREAT SYPHILIS 🌡️🦠

Julius Wagner-Jauregg was an Austrian psychiatrist known for his controversial treatment of neurosyphilis, a severe complication of syphilis that can affect the brain and nervous system.

Wagner-Jauregg observed that some patients with neurosyphilis who developed other infections accompanied by high fever sometimes showed improvement in their neurological or psychiatric symptoms.

This observation led him to develop malariotherapy—deliberately infecting patients with malaria to produce repeated episodes of high fever. The malaria was later treated with quinine.

His treatment was dangerous and controversial, but at a time when effective treatments for neurosyphilis were extremely limited, it represented an important therapeutic development. In 1927, Wagner-Jauregg received the Nobel Prize in Physiology or Medicine for his discovery of the therapeutic value of malaria inoculation in the treatment of dementia paralytica, a manifestation of neurosyphilis.

✅️ How Did Wagner-Jauregg's Treatment Work?

The treatment was based partly on the observation that Treponema pallidum, the bacterium responsible for syphilis, is sensitive to elevated temperatures.

By inducing malaria, doctors could produce repeated episodes of high fever. The fever and accompanying physiological and immune responses were thought to interfere with the syphilis infection and improve neurological disease.

However, the exact mechanism was not completely understood at the time.

It is therefore too simplistic to say that fever simply “broke down the barriers” protecting bacteria in the brain and allowed the immune system to destroy them.

⚠️ Was Wagner-Jauregg's Treatment Safe?

No. It was a high-risk treatment.

Patients were intentionally infected with malaria, which itself could cause severe disease and death. Some patients died as a result of the treatment.

Nevertheless, before antibiotics became available, malariotherapy offered some patients with neurosyphilis a chance of improvement when few effective treatment options existed.

This is an important lesson in medical history:

A treatment can be considered valuable in its historical context without being considered acceptable or appropriate today.

💊 What Was the Legacy?

Malariotherapy was eventually replaced by penicillin, which provided a much safer and more effective way of treating syphilis.

Today, deliberate malaria infection is not a standard treatment for neurosyphilis or severe infections. Modern antimicrobial therapy has made this dangerous approach obsolete.

Wagner-Jauregg's work nevertheless remains an important chapter in the history of medicine because it demonstrated how an unusual clinical observation could lead to a new therapeutic approach.

His story also reminds us that medicine is continually evolving.

What was once considered a breakthrough may later be replaced when better evidence, safer medicines and more effective treatments become available.

And interestingly, the idea of using fever therapeutically was not entirely new. Physicians had made observations about fever and disease long before Wagner-Jauregg.

The important distinction is that fever is part of the body's response to infection, but deliberately causing a dangerous infection is not a treatment strategy we should reproduce today.

I hope you learnt something new. 💊
